The 2025 Community Health Needs Assessment (CHNA) is complete. Community Hospital-Fairfax (CH-F) thanks the many contributors to the process from those who took the time to complete the survey to focus group participants.

The needs assessment is the product of research, focus groups and a community wide survey. The goal of the assessment was to determine what the greatest health needs were in the community and how Community Hospital-Fairfax can best meet those needs. 

The 2025 CHNA was very valuable and CH-F has used this data to develop the 2025 Implementation Plan.
